The information provided in these forms is collected and stored by Lake Macquarie City Council for the purpose of assessing and administering donated waste services. Access to the medical information is limited to only the officers required to receive and process the application and maintain the secure software data management systems. The keeping of confidentiality is required of these officers in accordance with the NSW Health Records and Information Privacy Act 2002.
If the donated service is approved, the address (without names or medical information) for which the approved services are to be provided is given to services providers (Council garbage collection staff and contractors). Garbage bins for approved additional donated services will be identified with an 'Additional fortnightly' or 'Weekly' sticker in the same way as paid additional services.
The information is stored at Council’s Administration Office at 126-138 Main Road, Speers Point, NSW, and may accessed or updated by applicants on request. If all or part of the information requested is not provided, the Council will be unable to assess the application. Incomplete information may impact Council’s ability to make a fair decision on the application.
